I am requesting permission to incorporate (insert specifically what will be used here) as part of a presentation to be used solely in my classroom lecture at Palmer College of Chiropractic.
Insert a brief statement here explaining how and why the materials will be useful. For example: I am in the process of changing my presentation methods to include more computer based materials. As part of this process, I have discovered that your images will better illustrate some of the concepts that I cover.
Your materials will be used on one personal computer that I use for classroom presentation; copies will not be made and my product will not be on a network. Other than enhancing classroom presentations, I will not financially benefit from this process.
I have signed and enclosed two copies of this request. If you agree to grant permission as outlined above, please sign both copies keeping one for your records and returning the other to me at the Palmer campus. Thank you for your consideration in this matter.
