SUPPLEMENTAL LIFE INSURANCE
When am I eligible?
The first of the month following date of hire.
How much Supplemental Life Insurance can I initially purchase?
Upon initial enrollment, your Guaranteed Issue amount for yourself is $150,000; $50,000 for
a spouse and up to $10,000 for each child. If you do not elect the insurance when you are first eligible or you chose to elect more than listed above, you will need to complete an Evidence of Insurability form.
The maximum amount of coverage you may elect for yourself is 5 times your salary in increments of $10,000, not to exceed $500,000. A spouse is eligible for up to 100% of employee coverage in increments of $5,000. Each dependent child is eligible up to 100% of employee coverage amount in increments of $2,000 to a maximum of $10,000.
In order to purchase life coverage for your spouse and/or children, you must purchase life coverage for yourself. AD&D coverage is also available.
How much does it cost?
The unit cost of Supplemental Life Insurance varies for employees and spouses according to age. The benefit brochure includes a table showing the monthly premium cost per age group.
How do I name my beneficiary?
Beneficiary designation is made on the enrollment form.
Can I change my beneficiary, if so how?
You can change your beneficiary at your discretion, however, you must complete a Change of Beneficiary Form. Contact

Can I take this life insurance with me if I terminate employment with Palmer College of Chiropractic?
Yes. You have 31 days from termination of employment to
port your group coverage to an individual policy, up to the current policy amount.

Does this policy have a Terminal Illness Benefit?
Yes. The terminal illness benefit will pay a lump sum - 75%
of the coverage amount in force or $50,000, whichever is less - if you become
terminally ill and your life expectancy is 12 months or less.
